I agree that I am responsible for the payment of all fees for consultations, testing and/or other treatments provided by The Lung Health Clinic. A fee schedule will be sent to me at the time I make my appointment.

I will be prepared to pay for the consultation and any testing undertaken immediately after being seen.

If I cannot attend my appointment, I will notify The Clinic at least 48 hours beforehand, to enable the appointment time to be made available to another patient.

If I cancel less than 48 hours in advance or do not attend my scheduled appointment, I will pay the full fee for the consultation and any booked tests. This fee will not attract any Medicare rebate.
